OVERNIGHT BUILDING SUPERVISOR
AHRC's Shoreham ICF has an immediate opening for an OVERNIGHT
BUILDING SUPERVISOR in the Shoreham Intermediate Care Facility. The
position involves supervising staff and services of ICF (Intermediate Care
Facility) building(s) throughout the overnight shift; ensuring that a
homelike environment is maintained, providing dignity and respect
throughout the overnight hours. Candidate must have a Bachelors Degree
in a human services field and a minimum of two years supervisory
experience working with adults with developmental disabilities.
This is a full time overnight position, 40 flexible hours per week. Under the
supervision of the Assistant ICF Director, an employee of this class is
responsible for the overnight operation of the ICF. This individual is
expected to exercise considerable judgment and initiative in carrying out
job responsibilities. Work will be reviewed periodically through evaluations,

CASE COORDINATOR (Shoreham ICF)
Full time/forty hours per week. Flexible work week. Responsibilities include
acting as the "Total Plan of Care" liaison between the Day Program, the
ICF, and the family. This entails communication, interaction and sharing of
information to provide coordinated, aggressive, consistent active treatment
and integrated programming for the program participants. Scheduling of
program participant quarterly reviews with Day Program. Reviewing
program participant goal plans and corresponding data sheets on a
periodic basis, as well as maintaining master goal lists, an monitoring "staff
actions" and "ongoing services". Interfacing with houseparents and
clinicians and completing and submitting annual Medicaid recertification
applications to Department of Social Services and resolving
Medicaid/Medicare positions.
Requirements: BA Degree in health related field; experience in the field of
developmental disabilities involving monitoring program documentation.

HOUSEPARENT / RESIDENCE MANAGER
Live-in, full time position available in an AHRC Community Residence
located in Suffolk County. Supervise developmentally disabled adults in a
homelike setting. Forty hours per week position with a private apartment.
Three years of experience required, residential setting preferred, plus
education in the field of mental retardation. Supervisory experience a plus.

REGISTERED NURSE
One nursing position is available to work with adults with developmental
disabilities in a day program setting at AHRC Shoreham ICF. This is a full
time position. Monday - Friday 8:00 a.m. - 3:30 p.m. (35 hours per week).
No mandated shifts, holiday, or weekend work. Responsibilities include
administering medications, providing first aid, and completing required

These positions consist of 40 hrs. per week, daytime hours M - F.
Responsible for developing and supporting participants to attain their
chosen outcomes and planned activities. An employee in this position will
be called upon to drive the program van on a regular basis as part of
community activities. The ideal candidate is energetic, responsible,
organized and flexible. Capable of applying good judgement in a variety of
settings.

Job Description: Assists the classroom teacher working with preschool age

NURSE (LPN) (Bohemia, Shoreham)
The Nurse provides the appropriate medical/nursing care required to
meet the needs of the developmentally disabled population. This individual
is responsible to review, coordinate, and implement medical care for each
participant. Their responsibilities include administering medication,
implementing physician recommendations, documentation in the
participants charts, and other assigned duties. The Nurse is an integral
member of the interdisciplinary team.
Minimum Qualifications: New York State Registered Professional Nursing
License OR New York State Licensed Practical Nurse License. Candidate
must possess and maintain a valid New York State Driver's License.

RECREATION AIDE
A part time weekday / weekend Recreation aide position is available at the
Shoreham ICF. Individual implements recreational programs of a
therapeutic and rehabilitative nature for the developmentally disabled.
Requirements: Candidate must possess a valid high school diploma or
equivalency, and have one year experience working with the
developmentally disabled; candidate must possess a valid NYS Driver's
License (and maintain it through duration of employment) and adhere to
the Agency Fleet Safety Driver's Policy and Procedure; must be certified
as an A.M.A.P. (Accredited Medication Administration Personnel) within six
months of appointment; must provide documentation of a current Mantoux
Tuberculosis test prior to beginning employment and yearly thereafter. A
criminal background check will be processed before assuming job

DIRECT CARE STAFF (ICF)
Assists and instructs developmentally disabled individuals living in an
AHRC Intermediate Care Facility (ICF). An employee in this class is
responsible for assisting and instructing program participants in the areas
of grooming and hygiene, bathing, eating, toileting, and all other activities
of daily living. Also includes assisting and instructing program participants
in recreational activities. Carries out training and treatment plans as
prescribed in self-care skills area and reports progress as required.
Participates in staff meetings and in-service training. Driving an agency
vehicle may be required for the purpose of program participant
transportation, shopping, recreation or other related ICF business.
Requirements: Candidate must possess a valid New York State driver's
license, valid high school diploma or equivalency, required annual PPD
Mantoux tuberculosis screening, and be willing to work holidays.
