Service Coordinators at AHRC Suffolk advocate for our participants
and provide linkage to community supports and services that are
consistent with the individuals needs and wants.
Our Service Coordinators, who have a minimum of a Bachelor of Arts
degree with many who have a Masters degree in a Human Service
field, are well trained and versed with social services, medical and
educational resources in Suffolk County.
The AHRC Suffolk MSC’s promote the concepts of individual choice,
individualized services and supports, and consumer satisfaction
while using a person centered planning process in developing,
implementing and maintaining an individualized service plan. Their
focus is on assisting each individual to achieve his or her unique
desires and dreams in terms of having a circle of friends, a home, a
job, enjoyable leisure and recreational activities, and access to
medical and clinical services. Monthly face to face meetings help
facilitate reaching the individuals’ desired goals.
